I understand that by participation in this program is used for emotional support and consultation. I will not be provided with medical attention or mental health diagnosis, such as from a psychiatrist or licensed psychotherapist. I understand that I am liable for my participation in this program and am responsible for my results therein.

Service Term - I understand that The Haven Cove reserves the right to decline or terminate the provision of services if my symptoms are severe and require medical attention, are harmful to myself or others or display deviant behaviors at any time during, before or after services.

Your sessions will not begin until all forms are complete. These forms serve as your contract agreement for services.
